  • This then places the clients on the Prioritization list/the City’s housing list
  • Successfully housed 20+ unhoused clients through housing programs and by personally taking them out to search for housing
  • Assist clients in obtaining entitlement services such as SNAP, TANF, SSI/Disability, Medicaid/Medicare, etc
  • I also assist in helping clients to obtain driver’s licenses, birth certificates and social security cards
  • Enter new client date into our HMIS system, along with obtaining and entering any updated information on active clients
  • Assist clients out of the office and provide daily outreach to those who are homeless
  • Collaborate with other agencies to advocate for the unhoused community
  • Successfully assisted in running our yearly Winter Outreach Warming/Sheltering Bus
  • Maintain clear and concise documentation, files, and records for all clients
  • Perform follow-ups with all my clients to ensure they are utilizing given resources and support systems to ensure that they maintain housing and while working to obtain permanent housing
  • Train new employees and currently supervise a staff of 4 employees

  • Provided 24/7 suicide prevention, crisis intervention services, and conducted mental health assessments to callers across the U.S
  • Applied the Applied Suicide Intervention Skills Training (ASIST) and V-Model when providing support to callers in crisis
  • The ASIST and V-Model are techniques by which the crisis worker explores and begins to understand the world of the client, and results in the crisis worker and client agreeing on a plan of action
  • Effectively triaged and answered incoming calls from 4 different crisis hotline centers
  • Maintained clear and concise documentation of client call notes which allows for immediate access to client records/assessments, resources utilized, intervention plans, and progress of treatment
  • Documentation should also protect clients’ privacy to the extent that is possible and appropriate
  • Successfully executed wellness checks to police departments for callers who are believed to be in imminent danger
  • Provided local and non-local resources to assist callers in crisis
  • This resulted in immediate access to social services to meet their current needs, regardless of where they are located in the U.S
