- I also assist in helping clients to obtain driver’s licenses, birth certificates and social security cards
- Enter new client date into our HMIS system, along with obtaining and entering any updated information on active clients
- Assist clients out of the office and provide daily outreach to those who are homeless
- Collaborate with other agencies to advocate for the unhoused community
- Successfully assisted in running our yearly Winter Outreach Warming/Sheltering Bus
- Maintain clear and concise documentation, files, and records for all clients
- Perform follow-ups with all my clients to ensure they are utilizing given resources and support systems to ensure that they maintain housing and while working to obtain permanent housing
