• Collects and documents medical history; measures and records vital signs; gathers, evaluates, and reports information regarding patient behaviors, complaints, or symptoms;
• Responding to and advising direct care providers with health-related concerns regarding individuals supported.
• Assessing individuals for injury and illness, referring to a higher level of care when needed and ensuring follow-up is completed.
• Reporting information regarding complaints, or symptoms to the individual’s PCP.
• Obtaining, processing, and handling lab specimens.
• Administering medications to include preparing and giving injections.
• Performing minor procedures and treatments; should have experience with PEG tubes, ostomies, and catheters.
• Provides individuals and families with supportive resources; educates individuals and direct care staff in areas impacting the individual’s health, wellness, and daily living activities.
• Assists in providing specialized health training and education to staff.
• Transports and accompanies individuals to medical appointments, urgent care or ER visits as needed.
• Updates electronic health record and helps to manage the EMAR.

• Assisting adults with intellectual and developmental disabilities in completing activities of daily living.
• Completing nursing tasks delegated by the supervising RN.
• Completing daily, weekly and monthly documentation outlining cares provided to those receiving services.

• Assure individuals health and safety.
• Administer medications or oversee individuals served in taking medications.
• Maintain required documentation.
• Address and manage disruptive/aggressive behaviors or situations. May need to use crisis intervention techniques.
• Able to work independently to assure health and safety needs of persons-served are met to include but not limited to completion of and follow-up on monthly health and safety inspections.
• Assist person-served in developing natural supports in their communities.
• Transport designated persons served to and from and assist at medical appointments, as needed.

A typical week working evenings and weekends might involve helping your client:

• Buy groceries and plan meals
• Keep the house organized
• Buy clothes
• Enjoy their leisure time
• Eat, bathe, go to the bathroom, and take care of themselves
• Get to and from work
• Enjoy TV time
• Go to restaurants, concerts, and other social activities

• Provides, assists with, or coaches individuals on personal care needs with limited supervision; provides crisis interventions, including physical intervention and restraint to ensure the safety of individuals; monitors and/or administers medication as prescribed; monitors and documents mental and physical condition of individuals.
• Teaches, coaches and provides guidance and support on various skills to enable productive lives of individuals; assists individual and family members to identify strategies or treatment options and participates in the development and modification of plans to regain ability to make independent choices and promote coping skills; develops and maintains a positive and effective relationship with service recipients, families, and other service providers, serving as the main point of contact.
• Organizes and participates in meetings related to care coordination; completes documentation for direct services provided to ensure timely and accurate completion.
• Transports individuals receiving services to appointments, activities, and/or work.
